Federal Tax Forms

The following forms are provided to employees or independent contractors by Tacoma Public Schools each tax year. Employee tax forms for current staff are available electronically in Employee Self-Service.

W-2 Form

This is an Internal Revenue Service tax form used to report wages paid to employees and the taxes withheld. W-2 forms are available to employees electronically by Jan. 31 each year. Staff have the option to have it mailed to them.

1099-MISC/1099-NEC Form

This is an Internal Revenue Service tax form used to report payment to independent contractors or vendors. 1099 forms are mailed by Jan. 31 each year.

1095-B/1095-C Form

This tax form reports health insurance coverage for an employee and/or eligible dependents. Forms are available electronically to employees each year through Employee Self Service (Focus ESS).

  • Form 1095-B verifies whether a former employee and eligible dependents, if applicable, were offered self-insured health care coverage (e.g., PEBB Uniform Medical Plans) through PEBB for one or more months of a tax year. Former employees with other PEBB coverage will receive their 1095 form directly from HCA PEBB
  • Form 1095-C verifies whether an employee and eligible dependents, if applicable, were offered health care coverage by TPS.
