The district will mail informational brochures describing the 2010 Healthy Youth Survey to all 6th, 8th, 10th, and 12th graders' families in September.
To help students grow up in healthy surroundings, we need to know more about the strengths and challenges in our communities and its programs for young people.
What is it?
The Washington State Healthy Youth Survey measures health risk behaviors that contribute to morbidity, mortality, and social problems among youth in Washington State. These behaviors include alcohol, tobacco, and other drug use, behaviors that result in unintentional and intentional injuries (e.g., violence), dietary behaviors, physical activity, and related risk and protective factors.
When is it?
During the week of October 18-22, 2010, Tacoma Public Schools will join others in the state to administer a Healthy Youth Survey to 6th, 8th, 10th, and 12th graders. This is a completely voluntary and confidential survey, designed to gather information to plan programs that support students in the schools and community. 2010 survey results will be available in late March 2011.

All opt out forms are due in school offices no later than Friday, October 8.

A parent or student may opt out of the survey in two ways:
Print the opt out form from the link above and mail or deliver it to your school's front office by Friday, October 8.
Families may also request an opt out form from each middle or high school office.
